Consultation and patient forms
Depending on the nature of your concern, a consultation will require between 15 and 45 minutes. Please ensure that you arrive a few minutes before your consultation to complete the necessary patient forms. For convenience, these forms can be printed and completed ahead of time and brought along to your consultation.
The hospital admission form can also be printed in advance and brought along with all other relevant documentation – medical aid card and ID – when you register at the hospital at least two days before the operation.
